Got it a few years ago with a complete package - inner plastic box, outer cardboard box, instruction manual, all that jazz. I always wanted a 1969-made watch (same as my age) and this was the closest I've been able to come to. It's perfectly wearable even today and even on my thick wrist :-)
I don't know the name of this model, time-span of manufacturing and I've never seen a watch like this anywhere, eBay including.
It looks like the Accutron Day Date AW, your dial is slightly different than the ad but the case is a match.
Is your case Gold tone or Stainless?

If it is Stainless then its not the AW, here is a Swiss ad showing it in Stainless with both a band & strap but no model designation for either. The one with the band has a Blue dial and it looks like the one with the strap might be yours.

Yes, this would be the watch - it has exactly the same blue details on a dial. I even have the stainless steel band - it's too short for my wrist so I wear an aftermarket flexi instead...
