In 1970 my high school faculty gave me a Bulova watch as top male student. A few years late it was stolen from a college gym locker. If I have Case #, box, and date code, can the model be determined. I do not recall what it looked like, as it has been 50 years since I have seen it. I cannot add this watch to the website as I have no pictures of the watch, or the watch itself.

Most likely no, we cannot. The serial (you call case) number doesn't identify a particular model. The presentation box you have was widely used in the 1960 and early 70's. You could peruse our data base by year in the mid to late 1960's (your date code indicates 1967), to see if anything looks familiar.

As both Neet and Geoff have said I think your best bet is to search through the adverts of this period. You might be in luck with an hexagonal case, as there aren't too many of those. I'll take a look through our 1967 Bulova linebooks for any matching case shapes.
