I did find two other example under google image search but the text was some kind of Asian text and would not translate. At least I know now it is not a one of a kind since I found others on a web search.
Very nice Accutron day and date. We should be able to find an ad for model/variant if we have one on site, as the dial is very distinctive. Looks original and complete, including band.
Someone told me that I had the case number wrong that it is on the inside of the case back not the outside. If this is correct I will change the input on the case number. The number on the inside is T-3422

Very nice condition watch; unfortunately I could not find an ad to give it a model number. Have to go with Accutron Date & Day. The T-3422 is the case number used to identify the case and parts for it; the case serial number you entered is the correct one.
I couldn't find an ad either. Accutron Day & Date from me too.
I dont know if this will help, a person on my watch forum had two of these. They did not know what they were called as a model either. But they had one for 1970 and the other was the same as I posted 1974.

This watch is beautiful. We gotta find an ad for it! Very distinct dial, so it should be easy to tie ad to watch if we can find one with variant or model name/number. I really like this watch!
