I've always had a soft spot for this case design.
I wasn't able to find a dial match in the linebooks, but did find the silver dial version.
Generic 1967 Bulova Accutron Calendar for now until we find the variant.

Thanks for this. I couldn't find that case and face design anywhere in any color. By the way, I measured the band width at the connection point and it's 17mm, but I can find no 17mm bands. Suppose I should go with a 16mm?

Disregard the watch band question. I found some 17mm bands.
