I have been trying to find this Bulova model without success. The date code on the back of the case, as well as the movement, is M5. I have looked at models in 1964, 1965 and 1966 without any success. Please help!

Hello everyone. Just another update from the creator of this post. I just sent a message to Bulova directly with photos, details and relevant content from this post, asking for help in identifying the model. I don't know how good they are at responding or their ability to identify unknown watches, but it can't hurt to ask. Anything I get back from them I will post here for shared knowledge. Maybe, just maybe, we'll crack this case and have another oddball Bulova identified!
